Possible meanings are 1) a young woman old enough to marry will become pregnant the natural way or 2) a virgin will become pregnant through a miracle or 3) Isaiah is referring to both options. Sometimes a prophecy is fulfilled once in a certain way then fulfilled again later in another way.

This emphasizes that within twelve years there will be an abundance of curds and honey to eat because the most of the people of Israel will be killed or taken as captives.
